Image Survey for Women with Hair Loss

Your opinions matter, in what is probably the first survey of headwear and wig use for women experiencing hair loss. We hope the results of this questionnaire will help us understand more about what women use to help them deal with their hair loss. Alopecia UK and Bald Girls Do Lunch® are interested in gaining a better understanding of what products people with alopecia use, so that they can improve the support they offer. They have joined forces with Suburban Turban , a specialist headwear supplier who will also be using the information provided in this survey to improve their headwear options for people with alopecia. They are offering a prize to a lucky winner. Please fill in your name and an email address to be entered into a prize draw. They have also kindly funded the costs for this survey.

Any information you provide will be treated with the strictest confidence and will not be shared with a third party or used to market any services to you unless you indicate otherwise at the end of the survey. All completed surveys submitted will be automatically entered in to our survey prize draw to win a colour consultation . Liz Scott, Image Consultant, will advise you on your personal colour palette by post (photographs will need to be supplied). Or there is the option for the winner to select hats from the current Suburban Turban range to the value of £100 as an alternative prize. Competition Rules

1. No purchase necessary to enter the survey prize draw.
2. No cash alternatives will be offered to the specified prizes.
3. Entry to the competition is restricted to one entry per person please.
4. Multiple entries will be disqualified.
5. Excessive multiple entries from the same email account pertaining to be representative of friends and family will also be disqualified at the judges discretion.
6. No automated entries or third party are permitted!
7. The winner will be chosen at random, from all completed and valid entries to the competition. Only fully completed surveys are deemed valid entries.
8. Applicants are welcome from outside the UK, although applicants must be aware that postal delivery times will effect the completion of the colour consultation prize.
